There have been several cases where players have died as a result of playing blackjack mulligan. In 1992, Michael Fagan, Jr. died after playing blackjack mulligan at the Las Vegas Hilton casino. Fagan was found with 17 blackjack cards in his pocket, and it was determined that he had died from an overdose of cocaine and alcohol.

In 2007, 37-year-old Randy Gardner died after playing blackjack mulligan at the Gold Strike Casino in Tucson, Arizona. Gardner was found with 17 blackjack cards in his pocket and it was determined that he had died from asphyxiation due to playing too tightly with blackjack Mulligan.
